*TDR insurance covers normal maintenance and accidental damage that is not cosmetic. IT MUST AFFECT THE HORN’S PLAYABILITY In event of theft a police report must be provided in order for your instrument to be replaced. This lasts until the rental contract is completed. MANUFACTURER'S WARRANTY: This guarantees your instrument from any defects in the manufacturing process. This lasts until your student graduates high school. This is not the same thing as the insurance.

There will be a $75 deductible for case or instrument replacement. Insurance will not cover neglect.

You authorize regularly scheduled debits/charges to your banking account/card. Your account will be debited the amount indicated. I understand and agree that as this is an electronic transaction, adequate funds must be available for withdrawal from my account by the payment due date. If adequate funds are not available for that month’s payment and/or paid in full within 90 days the instrument will go into collections status.

TO CANCEL CONTRACT - THE STORE MUST BE CONTACTED, AND THE INSTRUMENT MUST BE RETURNED TO OUR POSSESSION. THE SCHOOL OR BAND DIRECTOR IS NOT PART OF O’MALLEY MUSIC! AFTER THE INSTRUMENT HAS BEEN DELIVERED THE CONTRACT CAN BE CANCELLED ANYTIME AFTER THE THIRD PAYMENT WITH O’MALLEY AND JOHN PACKER BRANDS. ALL OTHER BRANDS OF INSTRUMENTS THERE IS A MINIMUM OF SIX PAYMENTS.
