INCLUDED IN PAYMENT: TDR (Theft, Damage, Replacement), insurance, finance charges, sales tax. *TDR insurance covers normal maintenance and accidental damage that is not cosmetic. In event of theft a police report must be provided in order for your instrument to be replaced. This lasts until the rental contract is completed MANUFACTURER'S WARRANTY: This guarantees your instrument from any defects in the manufacturing process. This lasts until your student graduates high school.

There will be a $50 deductible for case or instrument replacement.

You authorize regularly scheduled debits/charges to your banking account/card. Your account will be debited the amount indicated. I understand and agree that as this is an electronic transaction, adequate funds must be available for withdrawal from my account by the payment due date. If adequate funds are not available for that month’s payment and/or paid in full within 90 days the instrument will go into collections status.

TO CANCEL CONTRACT - THE STORE MUST BE CONTACTED, AND THE INSTRUMENT MUST BE RETURNED TO OUR POSSESSION AFTER THREE PAYMENTS HAVE BEEN MADE. Initial horn payment is NON REFUNDABLE. SCHOOL FACULTY ARE NOT EMPLOYEES OF O'MALLEY MUSIC.
